1️⃣ Is This Tissue Damage — or Just a Joint Imbalance?
This is the first question.
There’s a big difference between:
- A mechanical misalignment or subluxation
- A joint restriction without tissue damage
- A sprain, strain, tear, or structural injury
If It’s Mechanical Misalignment Only
When we’re dealing with a joint that’s simply restricted or out of alignment:
- We expect Fast Change
- We often see Immediate Relief
- Sometimes we see Complete Resolution

That’s the adjustment at its best — precise, specific, powerful.
If There’s Tissue Damage — Biology sets the Timeline
If ligaments, tendons, cartilage, or muscle are injured:
- Healing takes time
- The adjustment becomes supportive
- It improves mechanics and reduces stress
- But it is not an instant “fix”
And that’s where we have to be realistic.
If tissue is damaged, biology sets the timeline.
Be a patient patient.
2️⃣ Is the Misalignment Orthopedic or Neurologic?
Not all joint dysfunction is the same.
- Orthopedic-driven problems are structural and load-based.
- Neurologic-driven problems involve altered muscle tone, guarding, or reflex imbalance.
Sometimes the joint isn’t “stuck.”
It’s being held in dysfunction by the nervous system.
If that’s the case, treatment strategy changes.
We adjust — but we may also need to calm tone, retrain motion, or correct upstream interference.
3️⃣ Is This Joint the Problem — or the Compensation?
This is where most people miss it.
When I evaluate your knee, I’m also asking:
- What is the foot doing?
- What is the ankle doing?
- What is the hip doing?
- What is the pelvis doing?
Because many extremity problems are compensatory.
The knee may hurt.
But the foot may be driving it.
The shoulder may hurt.
But the origin may be irritated nerves in the cervical spine changing how the shoulder muscles fire and stabilize.
If we only adjust the painful joint without addressing the driver, the problem returns.
